This page is to explain some of the words and phrases we use on this website:

Assessment - this is the way in which we work out with you what you need to be happy, safe, healthy and doing well.

Care Plan - this comes after your assessment. It's where we write down what's going to be done for you, who and by when so as to meet your needs.

Corporate Parent - because your parents aren't able to look after you at the moment, the Council is acting as your "parent". This means that the people who work for the Council and the locally elected councilors are all responsible for looking after you.

Needs Led Assessment - an assessment that looks at what you actually need rather than what services already exist.

Pathway Plan - your Pathway Plan looks at the support you will need to live independently. It can help you think about what you already know and about what you need to learn. It is your plan for the future. It includes things like where you'll live, any education or training you might want, jobs, your money, any cultural or identity needs, your health and your lifestyle

LAC PACK this is the folder young people get when they come into care. Its full of info about Just us advocates, independent visitors all you need to know about being in care. Its also a place to store all your legal paperwork. Caz always throws in a few goodies as well, like pens, stickers, key rings, etc.
